NAD+

Also known as Nicotinamide Adenine Dinucleotide, IV NAD, NAD infusion, Injectable NAD+

NAD+ is a coenzyme essential for mitochondrial energy production, DNA repair via PARP enzymes, and sirtuin-mediated longevity pathways. Injectable and IV NAD+ bypasses gut conversion to deliver NAD+ directly to systemic circulation. Intracellular NAD+ levels decline ~50% between ages 40–60 — replenishing them is one of the most credible longevity interventions available.

Typical dosing
100–1000 mg · 1–5x per week (loading), then weekly to monthly
Administration
IV, IM, SubQ
Half-life
~1–2 hours (plasma); intracellular effects persist days to weeks
Level
beginner

How it works

Serves as a hydride acceptor in the TCA cycle and oxidative phosphorylation, and as substrate for PARP enzymes (DNA repair), sirtuins (metabolic and epigenetic regulation), and CD38 signaling. Restoring declining NAD+ levels reactivates mitochondrial function and stress-response pathways.

Benefits

  • Increased mitochondrial energy production (ATP synthesis)
  • Activates sirtuins — longevity-associated gene regulators
  • Enhanced DNA damage repair via PARP-1
  • Improved cognitive clarity and mental energy
  • Reduced neuroinflammation
  • Improved insulin sensitivity
  • Accelerated cellular recovery and reduced fatigue
  • Support for addiction recovery at high IV doses

Dosing

100–1000 mg · 1–5x per week (loading), then weekly to monthly

IV: 250–1000mg per session — slow infusion required. IM: 100–300mg per session. SubQ: 25–100mg. Beginner IV start: 250mg. Experienced: 500–1000mg.

Ranges reflect commonly cited research protocols, not a prescription. Dose with a licensed provider.

Reconstitution

Lyophilized powder. For IV: dilute into 100–250mL normal saline bag. For IM/SubQ: reconstitute to 100mg/mL with sterile water.

Common stacks

Reported side effects

  • Flushing and chest tightness during IV (rate-dependent — slow the drip)
  • Nausea during infusion
  • Abdominal cramping
  • Headache
  • Fatigue 24–48h after high-dose sessions
  • IM/SubQ generally well tolerated

Who it’s for
